AHRC Nassau provides OPWDD-approved Day Habilitation Services in over 30 locations across Nassau County. Our Day Habilitation RN will teach and guide the program's Direct Support Professionals and support our residents in remaining happy and healthy.
In addition, you will:
- Provide oversight and medical assessments of people that attend the Day Habilitation to ensure their safety and well-being.
- Provide continuous training and support to the staff for them to efficiently meet each person's medical needs.
- Administer first aid as needed or instruct staff on necessary medical interventions.
- Review all medical records and paperwork, including annual physicals and medical visit appointments on a regular basis and following doctor's orders as prescribed.
- Collaborate with the interdisciplinary team to ensure all medical concerns are addressed.
- Act as a liaison with the family, residences, and MD to coordinate care.
- Ensure that all medical documentation is comprehensive and conforms with agency policies and procedures and is maintained in the person's medical chart.
- Ensure implementation of all new medical policies and procedures by training staff and observation of practices.
We require:
- NYS Nursing License
- Good professional written and verbal interpersonal and communication skills
- Experience supporting people with intellectual and other developmental disabilities a plus
