Job Description: The primary duties of the Medication Reconciliation Pharmacy Technician is to provide support to the Hospitalist Physicians and Hospitalist Liaison Nurses by collecting and updating current/prescribed medication intake for admitted patients, in addition to, documenting in EMR - Epic, the patient's family and social history.
